Hormonal imbalances, particularly those involving testosterone, can significantly impact men's health. Testosterone, the primary male sex hormone, is crucial for muscle mass, bone density, mood regulation, and sexual function. When testosterone levels drop due to ageing, stress, poor lifestyle choices, or medical conditions, it leads to a hormonal imbalance. This imbalance can cause symptoms like reduced libido, erectile dysfunction, fatigue, mood swings, and decreased muscle mass.

Chronic stress, for example, raises cortisol levels, which can suppress testosterone production, says Hyderabad’s Trinity Care urologist, Dr Arun Sharma. He also mentions that obesity and certain medications can also disrupt the delicate hormonal balance, leading to low testosterone. Balancing testosterone is essential for overall well-being, affecting both physical and emotional health.

Understanding testosterone

Role in men’s health: Testosterone is produced mainly in the testes and is crucial for male reproductive and sexual function. It also affects muscle strength, fat distribution, bone density, and red blood cell production.

Normal levels: Testosterone levels typically peak during adolescence and early adulthood and gradually decline with age. However, other factors can lead to a more significant drop, leading to low testosterone, or "low T."

Causes of low testosterone

Ageing: Natural ageing leads to a gradual decline in testosterone levels, often starting around age 30.

Chronic stress: Elevated cortisol levels from chronic stress can suppress testosterone production.

Poor lifestyle choices: Lack of exercise, obesity, poor diet, smoking, and excessive alcohol consumption can negatively impact testosterone levels.

Medical conditions: Conditions such as type 2 diabetes, obesity, metabolic syndrome, and certain autoimmune diseases can contribute to low testosterone.

Medications: Some medications, especially opioids and corticosteroids, can lower testosterone levels.
Symptoms of low testosterone

Physical symptoms: It could lead to decreased muscle mass, increased body fat, reduced bone density, hair loss, and fatigue.

Sexual symptoms: Low libido, erectile dysfunction, reduced semen volume, and infertility are also impacted because of low testosterone levels.

Emotional symptoms: Low testosterone levels can lead to mood swings, depression, irritability, difficulty concentrating, and low energy.

Managing testosterone levels

Healthy diet: A diet rich in lean proteins, healthy fats, and complex carbohydrates supports hormone production. Foods like eggs, fatty fish, nuts, seeds, and leafy greens can be particularly beneficial.

Regular exercise: Resistance training and high-intensity interval training (HIIT) have been shown to boost testosterone levels. Regular physical activity also helps in maintaining a healthy weight, which is crucial for hormone balance.

Sleep: Quality sleep is essential for testosterone production. Aim for 7-8 hours of uninterrupted sleep per night.

Stress management: Chronic stress can lower testosterone levels by increasing cortisol. Techniques such as mindfulness, meditation, yoga, and deep breathing exercises can help manage stress.

Weight management: Maintaining a healthy weight is critical, as excess body fat, particularly around the abdomen, is associated with lower testosterone levels.

Medical interventions

Testosterone replacement therapy (TRT): For men with significantly low testosterone, TRT can be an effective treatment. It involves administering testosterone through injections, patches, gels, or pellets. However, TRT should be considered after a thorough evaluation by a healthcare provider, as it comes with potential risks and side effects.

Medications: In some cases, medications like clomiphene citrate or anastrozole may be prescribed to stimulate the body's natural testosterone production.

Addressing underlying conditions: Managing underlying health issues such as obesity, diabetes, or metabolic syndrome can help restore testosterone levels.

Long-term strategies for hormonal health

Regular check-ups: Regular monitoring of testosterone levels and overall health is essential, especially as men age.

Avoiding endocrine disruptors: Limiting exposure to chemicals that disrupt hormone production, such as certain plastics, pesticides, and personal care products, can help maintain hormonal balance.

Lifestyle modifications: Long-term lifestyle changes, including a balanced diet, regular exercise, stress management, and avoiding smoking and excessive alcohol, are the most effective strategies for maintaining healthy testosterone levels.
