For many, a urinary tract infection starts with a burning sensation when peeing, urgency and lower-abdominal discomfort. However, sometimes the problems turn severe, and include fever, chills and back pain. These can signal an infection that’s no longer confined to the bladder. Experts say early detection matters.
A urinary tract infection (UTI) is caused by bacteria entering the urinary system, most commonly Escherichia coli. UTIs can affect the bladder (cystitis), urethra, or ascend to the kidneys. In most cases, symptoms remain confined to lower urinary tract discomfort and resolve with treatment. But if bacteria travel upwards, they can cause pyelonephritis (kidney infection) and, in rare cases, spread into the bloodstream.
According to a JAMA Network Open study of over 11,500 hospitalised adults, bacteremia (bacteria entering the bloodstream) from a presumed urinary source was rare, occurring in about 1.4 per cent of patients. However, when it did occur, it was linked with serious complications, longer hospital stays and higher mortality, especially in patients showing systemic inflammation signs like fatigue or leukocytosis.

Furthermore, according to the American Journal of Clinical Nutrition, frequent consumption of foods like fresh juices (especially berry juice) and fermented milk products containing probiotics was associated with a reduced risk of recurrent UTIs in women, suggesting diet might influence UTI susceptibility.
Multiple studies warn that UTIs accompanied by high fever, chills, flank (back) pain, nausea, confusion or reduced urine output may indicate kidney involvement or systemic spread, warranting immediate medical evaluation.
Adequate hydration helps flush bacteria from the urinary tract, while dietary choices that support a healthy gut microbiota could indirectly reduce UTI risk by limiting the reservoir of pathogenic bacteria capable of ascending to the bladder or kidneys.

Age, urinary retention, catheter use, diabetes and structural abnormalities of the urinary tract are clinical risk factors that elevate the chance of complicated UTIs and systemic spread, factors clinicians monitor closely alongside symptoms.
Most UTIs are treatable and benign, but when bacteria enter the bloodstream or reach the kidneys, they can tip into life-threatening territory.
1. What are the common symptoms of a urinary tract infection (UTI)?
Symptoms of a UTI include a burning sensation when urinating, urgency to urinate, and lower abdominal discomfort. Severe symptoms can involve fever, chills, and back pain, indicating the infection may have spread beyond the bladder.
2. What causes a urinary tract infection?
A UTI is usually caused by bacteria, typically Escherichia coli, entering the urinary system. It can affect the bladder, urethra, or kidneys, leading to conditions like cystitis or pyelonephritis.
3. How serious can a UTI become if left untreated?
If untreated, bacteria can ascend to the kidneys, causing pyelonephritis, or even enter the bloodstream, leading to bacteremia. This can cause severe complications, longer hospital stays, and higher mortality rates.
4. How can diet influence the risk of recurrent UTIs?
Consuming foods like berry juices and fermented milk products containing probiotics may reduce the risk of recurrent UTIs in women, as suggested by studies. A healthy diet can influence UTI susceptibility.
5. What lifestyle changes can help prevent UTIs?
Staying hydrated helps flush bacteria from the urinary tract. Maintaining a balanced diet that supports gut health can indirectly reduce UTI risk by limiting pathogenic bacteria that may ascend to the bladder or kidneys.
