Mouth ulcers are a common and relatively harmless affliction, most often the result of stress — or from accidentally biting your cheek, a spicy meal or minor infections. The majority of small sores will heal on their own in between 7 and 14 days. But when a mouth ulcer doesn’t heal after two or three weeks, keeps coming back, or is becoming more painful, doctors say to take notice.

A mouth ulcer that refuses to heal could be a lot more than just an inconvenience. Dehydration could be one of the reasons along with underlying nutritional deficits, immune problems, chronic inflammatory disease or systemic diseases. In rare and severe cases, it can even be an indication of precancerous or cancerous spots in the mouth, especially among those who use tobacco products or drink alcohol, or individuals who have a compromised immune system,” says Dr Anil Kumar, Senior Consultant- Internal Medicine.

Left untreated, a chronic ulcer can discourage proper medical diagnosis and treatment of the underlying condition. Most are mild, but an early medical evaluation can rule out complications and provide prompt care. Kumar says that the importance of recognising early warning signs and seeking timely medical intervention to ensure oral as well as holistic well-being.

Causes Why Does a Mouth Ulcer Not Heal

Oral ulcers that do not heal frequently indicate a lack of iron, vitamin B12 or folic acid. Bad oral hygiene, poorly adapted dentures, sharp teeth and repeated trauma are other factors preventing healing. Hormonal abnormalities, repetitive stress and smoking will only further delay healing.

Underlying Health Conditions to Consider

These include diabetes, autoimmune diseases, gastrointestinal illness and suppressed immunity, which can all delay healing, doctors say. Recurrent ulcers could even be indicative of diseases, such as aphthous stomatitis or inflammatory bowel disease.

When to Be Concerned

An ulcer that lingers for more than three weeks, bleeds, is indurated at the margins, or if associated with weight loss, dysphagia or constant pain should be medically evaluated. These are signs that you should see a doctor or dentist right away.

What to Do: Doctor-Recommended Steps

Practice good oral hygiene, avoid spicy and acidic foods and drink plenty of fluids. Use any mouthwashes and gels that are prescribed by a dentist to help alleviate discomfort. Correct any nutritional deficiencies using diet or supplementation as recommended by a health care provider.
