'Not a freak accident, multiple systems failed': SIT findings on Noida techie Yuvraj Mehta's death in pit

According to investigators, serious shortcomings across departments responsible for urban planning, road safety, disaster response, and emergency coordination converged on the night Mehta lost his life.

The special investigation team (SIT) probing the death of Noida-based software engineer Yuvraj Mehta has concluded that the tragedy was preventable and stemmed from a chain of institutional failures rather than chance.

According to investigators, serious shortcomings across departments responsible for urban planning, road safety, disaster response, and emergency coordination converged on the night Mehta lost his life.

“The SIT is clear that this was not a freak accident. Multiple systems failed simultaneously -- road safety, drainage, disaster response, and command control. Had even one of these worked as mandated, the victim could have been saved,” a senior official associated with the investigation told News18.

Mehta, 27, drowned after his SUV plunged into a water-filled excavation pit in Greater Noida’s Sector 150 while returning home late on the night of January 16-17.

Dense fog and poor visibility, coupled with an unmarked construction site, turned an ordinary commute into a fatal incident. The pit had been dug for an unfinished project and left unsecured.

After the vehicle fell into the pit around midnight, Mehta managed to escape onto the roof of the submerged SUV. Unable to swim, he repeatedly waved his mobile phone’s flashlight and shouted for help, remaining trapped for nearly two hours before succumbing.

The SIT, constituted by the Uttar Pradesh government, issued over 22 detailed queries to the Noida Authority, police, traffic cell, fire services, and disaster management authorities. While extensive reports were submitted -- roughly 150 pages from the Noida Authority and about 450 pages from the police -- investigators said these failed to adequately explain the critical delays in rescue operations.

The team has shifted to Meerut for further analysis before submitting its consolidated findings to Chief Minister Yogi Adityanath. Administrative and legal action is expected once the final report is reviewed.

One of the central findings concerns the absence of safety measures at the roadwork site. Although the construction activity had official approval, investigators found no evidence of effective monitoring or enforcement. The SIT believes this lapse directly contributed to the accident.

“Files were approved, but execution was missing. Monitoring is not optional – it is the CEO’s statutory responsibility. The failure here is supervisory, not clerical,” a senior government source said.

Following this assessment, Noida Authority CEO Lokesh M was removed and placed on a waiting list. The SIT has also raised questions about district magistrate Medha Roopam, who heads the district disaster management authority.

Despite chairing a disaster preparedness meeting a day before the incident, she did not take immediate departmental action and visited the site only four days later, along with the SIT. She also did not meet Mehta’s family.

The conduct of first responders has drawn sharp criticism. Fire department personnel reportedly admitted they could not swim and lacked basic rescue equipment, while the chief fire officer did not personally supervise operations at the site.

“It is unacceptable that first responders reached a drowning site without swimming capability or rescue equipment. This reflects a deeper rot in emergency preparedness, not an isolated lapse,” an official involved in examining the fire department’s role said.

The SIT also questioned the police response. Although emergency helplines were alerted, the local station house officer did not reach promptly or effectively brief senior officers. Investigators are probing why specialised units such as the National Disaster Response Force (NDRF) were not called immediately.

“The local police treated it like a routine incident for far too long. The failure to immediately escalate to NDRF is one of the most serious questions the SIT has raised,” a senior police officer aware of the inquiry said.

Builders booked, family seeks accountability

The investigation has also focused on the role of private developers. Five builders -- Abhay Kumar, Sanjay Kumar, Manish Kumar, Achal Vohra, and Nirmal Kumar -- have been named in FIRs, and four have been arrested. The pit into which Mehta’s car fell was part of a basement excavation belonging to Abhay Kumar’s firm, Biz Town Planner.

“The pit was illegally left open and became a collection point for sewage and stormwater. This was a disaster waiting to happen. The responsibility of the builder is direct and criminal,” an investigating officer said.

Failures in traffic management have also been highlighted. The SIT flagged the role of Noida traffic cell general manager SP Singh, who is responsible for identifying accident-prone zones and ensuring installation of signage, reflectors, and barricades.

None were adequately present at the site. Drainage issues, including excessive sewage inflow from nearby housing societies, were also cited, with earlier instructions to install regulatory infrastructure allegedly ignored.

Despite more than 80 personnel from police, fire services, SDRF, and NDRF eventually participating in the operation, investigators believe the crucial early window to save Mehta was lost.

Mehta’s father, Rajkumar Mehta, has accused authorities of abandoning his son in his final hours. He said, “You raised your voice and supported my family and me in the tragic and untimely death of my son, Yuvraj, due to administrative negligence. You strengthened my resolve and guided this matter in the right direction, so that the negligent departments and their irresponsible staff could be appropriately punished.”

Recalling his son’s struggle, he added, “My son was very brave. Even after falling into the waterlogged pit, he struggled a lot. He gave us ample time, almost two hours, for the rescue operation. But the rescue team showed great negligence and left my son to God’s mercy. Whereas Yuvraj could have been easily rescued.”

While acknowledging that no punishment can undo the loss, Rajkumar Mehta said the family wants strict action so no one else suffers a similar fate. “We can never get justice for Yuvraj because he will never come back. But we definitely want that strict action be taken against those responsible for the negligence in this entire incident, and that it be ensured that no other Yuvraj suffers such a tragedy in the future,” he said.

Chief Minister Yogi Adityanath had ordered a high-level probe soon after the incident. The three-member SIT is led by the Meerut Divisional Commissioner, with the ADG of Meerut Zone and the PWD chief engineer as members.
