
If you are nearing 40 or are in your 40s, you may realise your body is not the same. This happens even when you are following the same routine and not doing anything unusual. This evident shift leaves many confused about what they need to do differently.
NHS doctor Amir Khan took to his Instagram account and explained that there are “biological changes” that happen in your 40s that no one really talks about or warns you about. Here are five such shifts that he shared in the video, along with suggestions on what to do.
Decline in muscle mass: Dr Khan pointed out that your muscle mass begins to decline in your 30s. So by the time you reach your 40s, the change becomes quite evident. It’s a process called sarcopenia. "Muscle fibre shrinks, strength drops slightly. and joints can feel stiffer because muscle is what stabilises and protects them. So if you feel weaker, less toned, or take longer to recover after activity, it’s not laziness, it’s just biology.” What to do? Strength train as you hit middle life.
Notice weight shifting to the abdomen area? The NHS doctor explained that even if your diet remains the same, fat distribution changes with age. “Estrogen and testosterone levels actually decline and that changes where the body stores fat: more around the abdomen, less around the hips and limbs.” Simultaneously, lower muscle mass also reduces resting metabolic rate. “So while the scale may not change dramatically, the body shape can,” he added.
This is one of the most common changes, as the lenses inside the eye become less flexible in 40s. This is called presbyopia. So if you find you're taking the menus or phone further away or needing brighter lights, it is a sign. Dr Amir called it “universal ageing of the eye,” which everyone experiences.

He said, “As estrogen levels slowly decline in mid life, the balance between estrogen and other hormones called androgens like testosterone shift.” Androgen stimulates coarse hair growth on areas like the chin and upper lip, so that’s why sudden darker wiry hair starts appearing. All this is part of hormonal change and there’s nothing to worry about.
This one’s applicable for both exercise or illness. The body takes more time to recover as “cellular repair processes, collagen turnover, and mitochondrial efficiency all gradually decline.” There’s also slightly higher baseline inflammation, which means the body needs more time.

Dr Amir said, “Many people in their forties notice they simply can’t stay up or function the next day like they used to. That’s partly our circadian rhythm changing with age and partly sleep architecture becoming lighter and fragmented. Melatonin timing shifts like area and deep sleep proportion reduces too. So late nights feel harder and sleep debt hits more.”
