
In a candid YouTube video, Aishwarya shared that she had long struggled with PCOS, hypothyroidism, and insulin resistance, conditions that made losing weight extremely challenging despite diet, exercise and daily step goals. She said she had tried traditional methods — including working with a trainer and nutritionist, increasing protein intake and walking 10,000 steps a day — but saw only body-measurement changes and not significant results on the scale.
Alongside her physical health struggles, Mohanraj also faced clinical depression, which she described as an intensely difficult time in her life. She said she felt like she was “spreading like ginger,” a phrase she used to capture how out of control and overwhelming everything seemed, and that her insecurities only grew stronger.
Her biggest focus became her double chin, which she called her “most insane and biggest insecurity.” At one point, the distress it caused was so consuming that she stopped making videos and withdrew from being online, because she simply could not bear to see her own image.
After bouts of plantar fasciitis and high uric acid, which forced her to pause workouts, she consulted her doctor and began weekly injections of Mounjaro, a drug originally developed to treat type 2 diabetes that has also been approved for chronic weight management. Mohanraj said she saw rapid results, losing about 1 kg in the first week and 4 kg in the first month, eventually dropping from about 74 kg to around 52 kg over six months.
While the transformation drew praise online, Aishwarya was forthright about the side effects and challenges she faced, including nausea, appetite suppression and noticeable hair fall associated with quick weight loss. She added, “That month, I lost 4 kilograms. I took this drug every week for 6 months. In these 6 months, my weight reduced from 74 kilograms to 52 kilograms. It’s rapid. It’s mad.” Over six months, she lost a total of 20 kg. However, she experienced significant side effects and costs. “The first month, I had intense nausea… I lost a lot of hair. And how f**king expensive this drug is… It’s ₹40,000 per shot,” she said.
Reflecting on the widespread belief that people should lose weight “the natural way,” Aishwarya acknowledged that while traditional methods like diet and exercise are generally ideal because they don’t involve side effects, they simply weren’t effective for her body given her health conditions.
She urged others struggling with body image to block out the noise from social media and comparison culture, explaining that ease doesn’t automatically make something wrong. She encouraged people to seek out trusted professionals, get the right support, and make changes that work for them, reminding them that this is their one life to live. Her core message to anyone considering change was straightforward: “Consult a doctor.”
