The condition, known medically as acanthosis nigricans, causes the skin to become darker, slightly raised and velvety in texture. It is most commonly seen at the back of the neck, under the arms or around the groin. While many people mistake it for dirt, pigmentation or poor hygiene, doctors say it often reflects what is happening inside the body.

Insulin resistance occurs when the body’s cells stop responding effectively to insulin, forcing the pancreas to produce more of it to keep blood sugar levels in check. Over time, this strain can lead to prediabetes, type 2 diabetes and other complications such as fatty liver disease and heart problems.

Doctors say acanthosis nigricans is particularly common among adolescents and young adults who may otherwise feel healthy. Because it is painless and develops gradually, it is often overlooked until blood sugar levels have already risen.

In India and other countries with high diabetes rates, clinicians are increasingly seeing the condition in younger patients. Sedentary lifestyles, frequent consumption of sugary drinks and ultra-processed foods, poor sleep and rising obesity levels are all contributing factors.

Importantly, acanthosis nigricans itself is not dangerous and is not contagious. Treating the skin alone with creams or scrubs does not solve the problem. Doctors stress that the appearance of the patches is a symptom, not the disease.

The first step, they say, is a medical evaluation. Simple blood tests can check fasting glucose, insulin levels and markers of insulin resistance. Early detection allows for lifestyle changes that can reverse or slow the process before diabetes sets in.

Doctors typically advise cutting back on sugary beverages, packaged snacks and refined carbohydrates, while increasing physical activity and improving sleep. Even modest weight loss can significantly improve insulin sensitivity in many people.

In some cases, medication may be required, especially if blood sugar levels are already elevated. As insulin resistance improves, the skin changes often fade over time, although this can take months.

Health experts say the key message is awareness. A dark neck is not a cosmetic flaw or something to hide. It can be the body’s way of asking for attention. Catching insulin resistance early gives people a chance to change course before more serious problems develop.
