Urologist explains the truth behind andropause or male menopause, when men hit their midlife shift

Men don’t escape hormonal changes with age — they just experience them differently. Known as andropause or male menopause, this slow dip in testosterone can quietly alter energy, mood, and overall health. With lifestyle stressors hitting harder today, recognising the signs early can help men age with confidence and vitality.

Menopause is considered a turning point in a woman’s life, a biological shift that signals change. But did you know that men, too, have their own version of it? It’s called andropause, or more commonly, male menopause. While it doesn’t come with the same drama, its effects can be just as real.

The fading testosterone

After a man hits 30, his testosterone levels begin a gentle yet steady decline, about one percent each year. “It’s not an abrupt stop like in women, but this slow fall can make men feel not quite themselves,” Dr Bhavatej Enganti, MS, MCh (Urology), Director of Men’s Health and Reconstructive Urology at the Asian Institute of Nephrology & Urology, Hyderabad, tells Moneycontrol.

He adds, “Symptoms such as tiredness even after good rest, mood swings, or loss of sexual interest are often dismissed as stress or ageing. But in reality, the body might be signalling that testosterone levels are dropping.”

Modern men and hormones

Modern life hasn’t been kind to men’s hormones. Long working hours, disturbed sleep, poor diet, alcohol, and stress are all silent culprits. Plus, lifestyle diseases like obesity and diabetes, and testosterone takes a further hit. “The cause of worry is that most men don’t talk about it. They normalise it, delay check-ups, and by the time they seek help, their overall health and relationships may already be affected,” warns Dr Enganti.

Symptoms of male menopause

Dr Enganti shares symptoms that might signal the arrival of  andropause

  • Persistent fatigue and low motivation
  • Loss of muscle mass or unexpected weight gain
  • Irritability, mood swings, or anxiety
  • Reduced sex drive or performance issues
  • Trouble sleeping or restless nights
  • Decline in focus and confidence

The expert strongly cautions against self-medicating or buying “testosterone boosters” online. “A proper evaluation by a urologist or andrologist is essential. A simple blood test can confirm if testosterone levels are genuinely low,” he advises.

If diagnosed, some men may benefit from Testosterone Replacement Therapy (TRT) — but only under strict medical supervision. In most cases, lifestyle changes can naturally revive balance:

  • Exercise regularly: Strength training works best for boosting testosterone.
  • Eat clean and balanced: Focus on lean proteins, healthy fats, and micronutrients.
  • Prioritise sleep: Aim for 7–8 hours of quality rest.
  • Manage stress: Yoga, meditation, or simple hobbies can do wonders.

Prevention tips:

Male menopause is a gradual transition, not a sudden stop, and it can be managed with awareness and care. Says Dr Enganti, “When men take charge early, they can stay strong, confident, and energetic well into later years. It’s about listening to your body before it whispers too softly to hear.”
