Period pain, also known as menstrual cramps, is a worrisome problem in many women. The pain can be in the form of mild discomfort to even severe cramps that make it difficult to perform daily activities with ease. Some women experience excruciating pain in the lower abdomen, back, or thighs, along with nausea, fatigue, or headaches, and have to rest for a day or two. Because the pain can be intense, many women take pain relief tablets to feel better and to be able to continue with their routine. While these medicines can provide quick relief, many women worry whether regularly taking tablets for period pain could affect their fertility in the long run.
According to Dr Prajakta Chaudhary, Fertility Specialist, Nova IVF Fertility, Mira Road, Mumbai, pain is seen when the uterus contracts to shed its lining during menses. These contractions are caused by hormone-like chemicals called prostaglandins. So, when the levels of these chemicals are high, the contractions tend to get stronger and cause severe cramps.

“To manage cramping that interferes with the woman’s daily routine, they often take pain relief medicines such as anti-inflammatory tablets that are usually advised by the doctor. These medicines work by reducing the production of prostaglandins, decreasing pain and inflammation,” says the doctor.
Mild or incidental cramps are generally normal and can often be alleviated with rest, heat therapy or occasional pain-relief tablets. Taking painkillers occasionally to relieve menstrual cramps does not usually impact fertility, doctors say. Some examples of these medications are the non-steroidal anti-inflammatory drugs which produce a decrease in inflammation and pain during menstruation, and they’re widely regarded safe when used in moderation and according to medical advice.
However, repeated self-administration of high-dose pain medication without consulting a physician is not recommended. Some medications going into the ovulation window may temporarily suppress egg release, but that is typically short-term (non-permanent) suppression and does not result in infertility.
Severe or chronic menstrual pain can be a sign of underlying medical conditions, such as Endometriosis, Uterine Fibroids and Pelvic Inflammatory Disease.
“If left untreated, these conditions can cause damage to reproductive organs or interfere with normal processes of reproduction, potentially leading to fertility problems. As a result, women with severe, progressive or recurring period pain should have a medical evaluation,” says the doctor.
