Why gout pain peaks at night? Early signs, triggers and treatment explained

Gout causes sudden, severe toe joint pain triggered by alcohol, purine-rich foods, trauma, or dehydration. Attacks show redness, swelling, and extreme tenderness, sometimes with fever. Diagnosis is by joint aspiration. Treatment includes NSAIDs or steroids, lifestyle changes, and urate monitoring to prevent recurrence

Sudden and severe joint pain that affects the first metatarsophalangeal joint (also called the toe knuckle) is known as gout. The episode usually begins during the night, and has been found to reach its maximum intensity within a few hours.

Risk Factors

An acute attack of foot is often precipitated by alcohol binges or purine-rich meals (like anchovies, shellfish or red meat). Trauma and dehydration may also trigger an episode of gout, along with sudden changes in the serum uric acid levels in the body. Pre-existing renal disease and metabolic syndrome have also been found to increase the risk of gout. Identifying the triggers is necessary for long-term management.

Symptoms and Signs

In between episodes of gout, the affected individual is in. The intercritical period, which may be asymptomatic in some cases. During the attack, persons experience marked tenderness and warmth, along with swelling and redness. The pain becomes unbearable and intense, and the person is unable to tolerate even light touches. If not treated timely, the attacks become more frequent.

Systemic Features: An acute episode of gout may also lead to systemic features like fever and chills, with malaise and leukocytosis. This may make it difficult to differentiate the condition from septic arthritis (clinically). The inflammatory cascade leads to such symptoms.

Joints Affected: Although involvement of the first metatarsophalangeal joint is common, gout may affect the ankles, knees, mid-foot and the wrists as well. Elbows and fingers may be involved in some people. In long-term cases of gout, the upper limb is affected and hand deformities become common.

How it is Diagnosed

The gold-standard method of diagnosis is joint aspiration, where the fluid is taken out from the joint and examined. The synovial fluid (present in the joints) shows inflammatory findings, but no organisms or pathogens. This aids in distinguishing gout from septic arthritis.

Rapid control of inflammation with non-steroidal anti-inflammatory drugs and corticosteroids may be recommended. The affected Indica must limit alcohol use and reduce the amount of red meat consumed. Regular monitoring of serum urate levels and avoiding the triggers may aid in long-term control.
